The Reablement Team has a unique opportunity for you to join the highly experienced, dynamic, progressive and expanding team. We are a successful team who have been able to ensure that 80% of our service users have remained independent in their own home without any formal support. We are also part of a team who were highly commended by the Health Service Journal in June 2018, for improving value in the care of frail older patients and in November 2018 part of a team highly commended for Integrated Discharge Planning and Reablement. The Reablement Supervisor role is part of an integrated Reablement Team and the wider Adult Health & Social Care multidisciplinary teams, to provide support with personal care and reablement to service users in their own homes and community through outcome focussed support. This is achieved by supporting individuals to regain, improve and maintain skills of daily living, promoting maximum independence.

Your next role

  • Driving in the community to numerous homes therefore; needing a full driving licence and access to a car.
  • To be available to work within a flexible shift rota of days or evenings. This is 07:00-15:00 and 14.30-22.30  Monday – Friday and an on call rota to cover weekends
  • Provision of support to Support Workers in “the field” including cover, induction, supervisions, appraisals, team meetings and ongoing learning and development.
  • Ensure that all risk assessments are regularly reviewed and updated to ensure the health and safety of both service users and the support workers. This could include environmental issues, medication, moving and handling, Home Line Plus and Telehealth care-this is not an exhaustive list.
  • To be proactive in liaising with other professionals/family/support workers.  Report concerns/incidents in line with all legislation, policies and procedures, including safeguarding concerns.
  • Screening of Referrals into the service.
  • Ability to learn IT systems that support the provision of the service.

About you

  • Be highly motivated and enthusiastic about providing a first class service to the residents of Swindon.
  • Ability to prioritise work load within a fluid service.
  • Knowledge and understanding of the Care Act and our obligations set out by the Care Quality Commission.
  • Have the ability to work independently, using initiative and as part of the wider Reablement Team.
  • Need a full driving licence and access to a vehicle whilst working.
  • Have experience and knowledge of caring for vulnerable adults with a wide range of complex needs.
  • Have experience of working in the community.
  • Have understanding of Health and Safety and Manual Handling issues.
  • Have understanding of the need to keep within professional boundaries.

Candidates must be fluent in the English language (as a requirement of Part 7 of the Immigration Act – for the effective performance of a customer-facing role).
