Live Well Health Improvement Coach
Job specification
Your next role
We are seeking enthusiastic Health Improvement Coaches to join the Live Well Swindon team, supporting people to take control of their health and wellbeing. Using an evidence-based approach, you will provide one-to-one and group support, helping people set achievable goals, build confidence, and make lasting lifestyle changes. Through motivational interviewing, behaviour change techniques, and signposting to local services, you will empower people to improve their health outcomes and strengthen their resilience.
In this role, you will deliver structured coaching sessions that are person-centred, safe, and effective. Meeting clients face-to-face in community settings, you will provide up to six sessions tailored to individual needs. By drawing on proven methods of early intervention and behaviour change, you will give people the tools and knowledge to manage their own wellbeing and stay connected to opportunities that support long-term health. Flexibility is key, as some evening and weekend work may be required to meet the needs of the communities you support.
As a Health Improvement Coach, you will also take an active role in community outreach, building trusted relationships with groups who experience the greatest health inequalities and are often seldom heard.
You must be able to drive or have access to a car in order to be available to support sessions and events around the town.
Your next package
