You are one of our integral Live Well Response Workers.  You will complete practical tasks for social workers in order to support people to live independently at home. Response Workers are also often the first person on the ground to deliver practical solutions for people being discharged from hospital. You will bring practical skills, compassion and tenacity to the role ensuring that Swindon residents without the support of a network of friends or family feel supported to return to their home after a hospital admission. You will also provide practical support to social workers completing tasks that help people retain independence and live happy healthy lives.

Your practical skills, good time management and full driving licence make you an ideal candidate. So too does your willingness to carry out tasks that involve manual handling (training will be provided) and work independently or as part of a team.  You will not be required to carry out tasks that involve personal care for Swindon residents.

Your next role

As part of a growing team, you will work with colleagues from across the health and social care system. The role involves partnership working with the Great Western Hospital. One day you may find yourself discussing patient priorities on the wards or moving furniture to make way for a hospital bed in a person’s home the next. No two days will be the same. You will be led by an experienced Project and Team Lead and colleagues in the wider team and will focus on ensuring that the Live Well offer is evidence based, person centred, well planned, safe and effective. You will always be curious to notice if someone you are working with might benefit from additional support or advice to ensure that residents, especially those facing the greatest inequalities, have positive healthy lives.

You must be fluent in the English language (as a requirement of Part 7 of the Immigration Act “for the effective performance of a customer-facing role”).

You must be able to drive and have access to a car in order to be available to support sessions and events around the town.
