Your next role
We are looking for innovative and motivated Live Well Navigator to work as part of Live Well, Health Improvement Services, Public Health, in support of individuals who live in Swindon who have one or more long term health conditions and would benefit from coaching and behaviour change interventions. The role of the Live Well Navigator is to work alongside the individual with a person- centred approach to improve their health, wellbeing and quality of life.
Within this role you will have the ability to build strong relationships with health, social care and the voluntary sector services in Swindon as well as finding ways to reach out to those facing greatest inequality and be creative and collaborative in your approach, inspiring and motivating lasting change.
You must be fluent in the English language (as a requirement of Part 7 of the Immigration Act “for the effective performance of a customer-facing role”).
You will be subject to an enhanced Disclosure and Barring Service Check and the ability to travel throughout the Borough will be required and access to a car
