An exciting opportunity has arisen for a Discharge Planner based at the Great Western Hospital within the Hospital Social Work Team as part of a multi-disciplinary team and in re-shaping Adult Social Care to meet the needs of our service users.

The Hospital Social Work Team assess and organise discharge services for patient’s requiring extra support before or upon returning home from hospital.

Your next role

You will be pivotal in information gathering and sharing to ensure safe and timely discharges as well as maintaining a true representation of current caseloads across the team. 

This is an excellent opportunity for an individual that has the passion to succeed and wanting to further develop new skills. The successful candidate will need to have a flexible approach to work in order to meet the demands within the acute setting.

You will have the opportunity to work with partners across the health and social care system and as an expert communicator you will liaise with colleagues across the organisation, ensuring compliance with relevant policy and legislation (The Care Act 2014).

You will need excellent clear communication skills dealing at all levels, maintaining tact and diplomacy at all times and hold an awareness of handling sensitive information.

You must have the ability to interpret levels of urgency/complexity when prioritising work and knowing when to escalate to a senior. 

In addition, you will need good IT skills and experience of working in a Health and Social Care setting would be an advantage.

This is a 7-day service and the working hours for this position are Tuesday to Saturday.

You must be fluent in the English language (as a requirement of Part 7 of the Immigration Act “for the effective performance of a customer-facing role”).
