You will be one of our motivated Health Coaches, providing one to one support for people in Swindon with one or more long term health conditions. You will be delivering a specific project which supports communities where people are seldom heard. Through building relationships, you will build trust and support people to work with services that can provide preventative interventions and provide opportunities for people to take advantage of opportunities to manage their health and wellbeing.  Your excellent knowledge of behaviour change and motivational interviewing make you the perfect candidate for this role. 

You will be working with individuals, providing opportunities to overcome barriers, engage with services, benefit from appropriate support and create new positive networks that improve resilience and self-efficacy. 

You must have strong partnership building skills to nurture relationships with colleagues across the voluntary and community sector to ensure that Swindon’s rich and vibrant community life, is accessible to all.

Your knowledge of early intervention as a means to improve health outcomes alongside an understanding of behaviour change models makes you an ideal candidate for this role.

Your next role

You will be delivering a one to one or group-based interventions to people who are referred in by Community leaders.  You will work closely with the Project and Team Lead, Programme Manager along with colleagues in the wider team and partners across the health and social care system and will focus on ensuring that the Live Well offer is evidence based, person centred, well planned, safe and effective.

You will find ways to reach out to those facing greatest inequality and are creative and collaborative in your approach, inspiring and motivating lasting change.

You must be fluent in the English language (as a requirement of Part 7 of the Immigration Act “for the effective performance of a customer-facing role”).

You must be able to drive or have access to a car in order to be available to support sessions and events around the town.
